Why Runners Feel It in the Knee

What the body does: repetitive single-leg loading, thousands of cycles per run, often on the same surface at the same pace.

Where it shows up: knee, shin, hip, Achilles, plantar surface of the foot.

Why it happens: most running complaints are volume and load-distribution problems rather than single-event injuries. When the hip and glute aren't contributing their share, the load moves down the chain and the knee absorbs what the hip should have. This is why treating only the painful knee tends not to hold.

Worth naming: the "it went away and came back" pattern is nearly universal in runners, and it's the strongest argument for looking above and below the pain.
