Through the contact form: your name, phone number, email address, and whatever you choose to write about your symptoms, injuries, or health history.
Please note: information you type into the form is transmitted over the internet and stored by our website provider. It is not covered by the protections that apply to medical records. Share only what you are comfortable sending by ordinary electronic means. Anything more detailed is better discussed by phone or in person.
